How do I get rid of existing questionnaire?

I have uploaded some “junk/test” questionnaires and now I can’t get rid of them … ?

We left the VA without that option, we included it already (in account reviews) and we’ll push it to VA’s probably next week.

that is how it looks on account reviews (very similar to VA)
